We offer a single point of access to the Mind Matters Surrey IAPT service across the county. Individuals can be referred by their GP, or they can self-refer by using our online portal.

Please complete the electronic referral form available on your GP surgery IT system or download this form to print and send to us. Please tick the ‘opt in’ box if your patient has agreed to contact the service, alternatively if this box is left blank we will initiate contact with the patient.

Completed referral forms, along with a PHQ9 and GAD7 score, should be sent to:

How quickly will my clients be seen?

Upon receipt of the referral, we will contact individuals within three working days via telephone, letter or text message. They will be offered an assessment and a decision to treat within 10 working days.

If we consider that treatment is appropriate, this will start within 28 working days of the referral. We may also make referrals to other agencies such as employment support services where appropriate.

You will be advised of your patient’s journey through the service and at discharge.
